VolView Insight: A Seamful Integration for Clinical AI

We introduce VolView Insight, a software platform designed to unify imaging and clinical data into a single interface. For clinicians, it provides an integrated view of images, patient records, and AI-assisted workflows. For researchers and developers, it offers a realistic testbed for deploying multimodal models in a setting that mirrors clinical reality.

Building on the Seams

Integrating technology into clinical workflows typically aims for a seamless experience. However, blending smooth UX with healthcare regulatory compliance, image viewership, and multimodal analysis can create a fragile and opaque system. VolView Insight instead builds on the seams, i.e. the essential connection points between systems, to provide robust capabilities.

VolView Insight enables the integration of an open-source radiological viewer at its core, along with a SMART patient data source and a modular Python backend for AI driven analysis. This provides the infrastructure to choose which patient data are ingested into a model or sophisticated AI pipeline and then display results onto the diagnostic screen. Consequently, VolView Insight allows for training deep learning models and their deployment within the clinical viewer. With access to the vast JavaScript ecosystem in the front-end, and with little added customization, VolView Insight can invoke API calls to cutting-edge medical-domain LLMs (such as MedGemma) and securely provide the context of an underlying EHR system, imaging data, or even allow for real-time chat between the clinician and AI.

Compliance by Design

A major focus of engineering has been enabling compliance with evolving healthcare interoperability and security standards:

• USCDI (2020) & US Core: Ensure access to essential patient data aligned with national data infrastructure priorities.
• HL7 FHIR: Standard API protocol for seamless EHR integration, as advised by the 21st Century Cures Act (U.S. Congress, 2016).
• SMART on FHIR: Enables provider and patient-authenticated PHI access, supporting the regulatory push for secure health data exchange.

This infrastructure allows our system to retrieve and combine DICOM imaging, clinical notes, labs, diagnoses, and timelines, all in real time and at the point of care.

Using VolView Insight

1. Log in and authorize access to patient data.
   • A clinician logs into the underlying EHR for credentialed access.
   • A researcher connects to the FHIR server containing experiment data.
2. VolView Insight loads patient images and EHR data in the web interface.
   • The user has the full power of the VolView radiology viewer to visualize patient data and markup images.
3. Perform an analysis on the combined imaging and health records. Custom backend AI models can be integrated for plug-and-play multimodal inference, leveraging both images and structured data.
4. Visualize, interpret, and utilize the results in context along with AI-generated insights. Results can be overlayed onto images and rendered to the clinician in the Analysis Panel.
